Case study: Dog impaled on wrought iron fence

Abstract

Short history:

Impaled on wrought iron fence for 3 mins. Operated on to try and close the wound within 60 mins of event happening. Deep puncture to thigh area and +++ skin degloved from chest/abdominal area.

Small canine - Podenco, Male 3yrs (neutered)

How would you like us to help?:

We are planning to partially close the wound by undermining local tissue on Wednesday. I would like help on how much tissue we can undermine to use safely and how much we can expect to close the wound by. How to dress the area initially where the skin will be pulled over (without tension) and how to improve granulatioin and epithelialisation of the part left to heal by secondary intention.

 

Management to date: 

Surgery Day 0 - tried to close wound. No too much tension.
Day 1 - placed drains sutures holding well. No sign of necrotic tissuein
Day 2 - removed drains. sutures fine. no tension
Day 3 - +++ exudate devloping
Day 4 - Skin flaps starting to die. Removed under GA and debrided area. Kaltostat dressing
Day 5 - wound bed looks healthy. redressed with hydrogel dressing.


The wound on the lateral aspect of the thigh was impossible to close and very deep with some muscle damage internally. There was only a small entry wound but the iron spike had penetrated all the way through and almost out of the skin on the causal aspect of the thigh. We had to leave it to heal by second intention but impossible to dress even with stay sutures.
As soon as the pet shirt with leg attachments arrived it prevented him licking or interfering with the wound and it started to heal much more quickly.
Dressings stayed beautifully in place under the suit.
